ESA Housing Renewal Questionnaire

We cannot assist with reptiles, barnyard animals, monkeys and other non-domesticated animals not considered common household animals.

    If you are interested in training your animal to become a psychiatric service dog (PSD), a PSD letter helps to establish whether you have a qualifying condition under the ADA. PSD letters are available as a discounted add-on if you purchase an ESA plan.
  • Are you considering training your dog to become a psychiatric service dog? A PSD letter can provide you with supporting documentation. A PSD must be individually trained to perform tasks relating to a mental health disability. Under recent U.S. air travel rules, ESAs are now limited on flights but PSDs can still board. A few things to note: A PSD letter does not in any way confirm, verify, certify or represent that you own or will own a service dog. Neither the healthcare professional you work with nor ESA Doctors is able make any representations or certifications about the qualification of your current or future animal as a service dog or its training or temperament. It is ultimately your responsibility to determine whether your canine meets service dog requirements and to make certifications to third-parties (including airlines).

  • Notice Regarding Flights: As a result of rule changes by the U.S. Department of Transportation, many airlines are no longer accommodating emotional support animals. Some airlines will still welcome emotional support animals onboard. Please check with your airline directly prior to booking a flight to see what their latest policy is.

    It is recommended that ESA letters be renewed annually. Landlords typically require an ESA letter that is current. We recommend opting for automatic renewal to ensure your ESA documentation doesn’t accidentally expire and to lock in your rate – you can always cancel at any time before you complete your renewal questionnaire for next year. Your renewal will be automatically billed next year at the locked in rate.

    Landlords often require their own forms in addition to the ESA Letter. You can add this service to your order so their form can be promptly completed by your licensed health professional if requested.

    If your landlord does not have their own form and this service is not needed, we will issue a full refund for the Extra Documentation fee.

    Choose this option if your order is time sensitive and you need your letter ASAP or by a specific time. All Expedited Service orders, including plan, are final and are not eligible for a refund.
